As part of our service, we provide you with the Ages & Stages Questionnaires, Third Edition (ASQ-3). This is a wonderful screening tool to help you keep track of your child’s development. Because your child’s first 5 years of life are so important, we want to help you provide the best start for your child. The questionnaire will be provided to you every 2 to 6 months depending on your child’s age. Also, it will include questions about your child’s communication, gross motor, fine motor, problem-solving, and personal-social skills.
The ASQ was developed to help provide social-emotional screening and accurate developmental information for children between birth and 6 years old. This tool is designed to help you keep an accurate record of your child’s development. It will also help catch any delays in children’s development. The process is fairly simple, select a questionnaire, complete it, and then score it. Make sure to discuss the results with a professional.
Questionnaire To Help Track Development
If the questionnaire shows that your child is developing without concerns we may provide some ideas about how to encourage your child’s continued development. You will then receive the next questionnaire at the appropriate time if needed.
If the questionnaire reflects some possible concerns we will contact you about getting a more involved evaluation for your child. It is important to us that we are able to support your child’s growth and development at an early age and prior to Kindergarten. Please rest assured that information will only be shared with you. Without your written consent your child’s results will not be shared with any other agencies or support staff.
